In this issue we discussing two topics, one is relevance of DNA PCR in disease diagnosis and other is basic steps of In-vitro Fertilization. We have realized that the workshop dates for Ovulation induction and sperm processing was clashing with Bhaidooj and hence it was practically difficult for the participants .We have postponed it and the new date is 3rd April Sunday 2005. I request once again to participate with us with your interesting cases, we will feel highly obliged

1.DNA PCR a new way of diagnosing diseases

DNA is the basic and core structure of any living things be it an animal or a human or the bacteria or virus.
We all are identified by our DNA structure. Although all the humans have same DNA to make them Homo sapience but each individual is having one specific type for himself and he/she is identified by that specific type (DNA fingerprint). Hence each individual is having a different DNA morphology. Because there are innumerable different humans so will be innumerable morphology, thus we are polymorphic and this characteristic of DNA is called Polymorphism.

Polymorphism (difference in DNA) may not be causing any disease (silent) or it may cause disease (it speaks) .If it causes a disease then the disease is known as genetic disease.

In other way, if we can identify that disease causing polymorphism by any means then disease can be diagnosed even before the appearance of actual disease, (like thalassemia, sickle cell anemia etc by doing prenatal sampling). This method is called DNA diagnosis of that particular disease, because it identifies the actual defect hence the diagnosis is more accurate.

If the polymorphic DNA (disease causing) is less in amount then diagnosis is difficult. Now if we have any method to make multiple copy of that DNA then the things will be easy. Technique of making multiple copy of that DNA is called PCR (polymerase chain reaction).

As all the organisms have their particular DNA pattern, if any of it enters into our body then we can track them if we know their DNA pattern. Thus bacteria of tuberculosis or parasite of malaria or viruses like hepatitis B or HIV can be tracked and if they are tracked then we can say to the extent of 100% that particular person is suffering from that particular disease. If the disease-causing organism is in very small in number then their DNA has to be copied means they also need PCR.

Thus DNA PCR is very useful and novel technique for diagnosing various genetic diseases and infectious diseases.

2. Steps of IVF cycle

  1. Downregulation of ovaries for 10 –14 days started during the luteal phase of the previous cycle.
  2. Gonadotropins (FSH or human menopausal gonadotropins ) generally in higher doses used in patients to achieve superovulation
  3. Monitor follicular response : usually by Sonography and/or estradiol
  4. Oocyte collection under ultrasound guidance, through the vaginal wall. Done under propophol or other sedation.10 –14 minutes procedure. The Oocyte yield is usually 90%. Some cases may require laparoscopic aspiration.
  5. Sperm samples are provided on the same day of Oocyte collection .( sperms may be cryopreserved earlier also )
  6. Oocyte and sperms are cultured overnight and, if fertilization has occurred, the embryos are further cultured.
  7. Embryo transfer performed using a fine catheter through the cervix into the uterine cavity 2-3 days postferilization
  8. Luteal support using progesterone is followed by pregnancy testing.
